Thanks for the reply. The error that I get from the Disk Copy
Applescript
(ftp://ftp2.sourceforge.net/pub/sourceforge/d/de/debian-imac/MakeDebianFloppy.sit)
is that <file> is not a valid disk image. Separately, I tried to open
the image in OSX and it faild there also. I fixed and confirmed the
file type and creator using ResEdit, but it did not make a difference.
Of note the boot.img file worked perfectly the first time.

Hehe, in the floppy days, we didn't use to "burn" to a floppy :)
so I think my dowload procedures are correct.
Any help or direction to a valid root.img file would be welcome.
What exactly are the symptoms of your failure ?
