In the out-of-the-box configuration of 'kdm', the "Start New Session"
feature found in the Lock/Logout-Applet and in the KDE start menu is
missing.
The source of the problem are commented out entries in
'/etc/kde3/kdm/Xservers', which looks as follows after the package has
been installed:

The additional server lines (at least one) ':1', ':2', ... must be
uncommented in order for the "Start New Session" feature to work.
This should be the standard setup for the 'kdm' package.
(The feature is also missing in the most recent 'kdm' package from
'unstable', which I have verified by hand.)

This bug only affected KDM in KDE 3 and KDM4 now supports "switch user" out-of-
the-box
The previous entries in /etc/kde3/kdm/Xservers have been replaced by the
following line in /etc/kde4/kdm/kdmrc which is present on a clean install:
