After tar xzvf the .orig.tar.gz, you have to rename mgetty_0.99.2.orig directory to just mgetty_0.99.2, then you can apply the patch by doing a "zcat mgetty_0.99.2-6.diff | patch -p0", and then you have to remove all the .orig files. But you might better download the .dsc file from the same directory you downloaded the .orig.tar.gz and the .diff.gz and do: dpkg-source -x mgetty_0.99.2-6.dsc And, voila, dpkg-source does everything for you. This should work with all the new packages. Remember to install the dpkg-deb package (which contains dpkg-source and other tools).
