Um... I noticed it detected only 512k ram. You are specifying 1024x768 at 16bpp color depth... are you sure there's enough memory for it? I've got 1024k vidram, and at 16bpp, all I can get is 800x600. Try dropping to 8bpp and see if it finds a mode it can use.
