Not necessarily. Take a look at the man pages.
I am using X from the 3.3.6-7 xfree-common package and have a 3 buttons 
Logitech mouse.

[03:01:06 /tmp]$ grep Mouse /var/log/xdm.log 
(**) Mouse: type: Microsoft, device: /dev/ttyS0, baudrate: 1200
(**) Mouse: buttons: 3
