/usr/lib/menu has lots of files you can use as templates
cp one to /etc/menu, tweak it, run (as root) "update-menus"
With KDE you may need to force the issue with...
rm -rf ~/.kde/share/applnk
to force the DB to be rebuilt, or maybe just restart KDE.
