Do you have /etc/tcp.smtp.cdb?
It is a database (sort of - a ``constant database''), used instead of
/etc/hosts.*. Read the qmail FAQ (http://cr.yp.to/qmail/faq.html)
or Life  With qmail (http://www.lifewithqmail.org/).
