Firefox & Co. have TWO independent margin setting systems:

1) "Margins & Header/Footer" in the File, Page Setup menu. Setting
   is in millimeters. This controls the location of the text on
   the page.

2) "Properties" in the File, Print menu. You can set the "Gap from
   edge of paper to margin" in inches. This controls the location
   of the headers/footers. If the values are too low, the headers/
   footers run off the page. This seems to be the default (the
   default values are too low, namely 0.04; 0.2 or 0.25 is
   better).

   But if the values are too high (and it seems you set them at
   maximum) the headers/footers can get lost "under" the
   text. In Firefox printing *text* apparently has priority over
   printing *footers*. If they overlap, you only get the text. Of
   course this happens especially if your top or bottom margin in
   the Page Setup is too small.
