This problem has been resolved by Helge at cups.org. The solution is at http://cups.org/newsgroups.php?s19897+gcups.general+v19906+T1 and the whole thread ("PostScript problem with Xerox Phaser 6280DN") is at http://cups.org/newsgroups.php?gcups.general+T1 It turns out that acroread and openoffice, given Xerox's Linux driver for the Phaser 6280DN, generate non-conforming PostScript that gives errors at the printer or when viewed by gv. Helge's solution involves adding just one line to the PPD file (/etc/cups/ppd/Phaser_6280DN.ppd in my case) and a file named oopstops. In Etch, oopstops is already present as /usr/lib/cups/filter/oopstops (part of the cups 1.3.8-1+lenny7 package). Just adding the one line to the PPD file (and restarting cups) succeeded in my case.
