Couple of possibilites:
- Hard disk is failing
- Insufficient power available for your hard disk, causing it to spin up
then spin down again
- Controller error
- Faulty connection or SATA port

The more likely possibilities are 1 and 3.

If you can get another hard disk to test, that will narrow down the
possibilities.
