I had the same problem on my Debian Wheezy box up to about two days ago. 
Apparently, during an upgrade the ~/,kde.share/apps/kaffeine/xine-config file 
was 
replaced with a default configuration file. I located the line labeled 
`#audio.driver: auto`, removed the comment, changed auto to pulseaudio and it 
started working again without needing a reboot. Just make sure to close 
Kaffeine before editing the file. For some reason, Kaffeine does not use the 
configuration in the .xine subdirectory.
