I've been using cups directly as previously described, not through any other interface. The good news is that in implementing your first suggestion I realized that I'd always been using the "recommended" Foomatic-Postscript driver in cups. I noticed that there was a gutenprint driver for this printer - and it _works_ with that driver! So it seems as though the Foomatic driver is broken, at least on this system.
