Volatile Submit Page Problem

Hi,

I submitted a mirror to http://www.debian.org/volatile/submit a week or 
two ago, and another request yesterday. When I hit submit, I received a 
blank page both times. Can someone check if the submit script is working 
and whether or not my submissions were received (they would have been 
for mirror.csclub.uwaterloo.ca).
