This happens quite often, but never during continuous single-user use, so I think it is triggered by VT switching or possibly upon resuming. The symptom is either the monitor displays an "out of range error" or the screen is frozen, with only the mouse moving - impossible to click on anything, switch VT etc. I can however ssh into the machine and thus see that xorg is occupying one cpu totally.
