I received an email referring me to <https://bugs.edge.launchpad.net/ubuntu/+source/xkeyboard-config/+bug/400718>, which describes the symptoms accurately. If I disable "Alt is mapped to Right Win, Super to Menu", everything works correctly. I hadn't noticed this previously because this is a ThinkPad R60, which doesn't have a Right Win key.
