But having trouble understanding the Headers test and how its used

If want it to look for "X-XtremeMailControl: Warning" and add a weight
of 5 to the message.  Is the following correct

HEADERS         5       CONTAINS        X-XtremeMailControl: Warning

Also does anything have to be added to the junkmail file
For filters, HEADERS will check the entire headers for a specific word/phrase. So the line you show above would catch a header "X-XtremeMailControl: Warning" (assuming that the line was part of a filter that is set up properly). You would need to have an action listed for the test in the \IMail\Declude\$default$.JunkMail file, or Declude JunkMail will not take any action on E-mails that fail the test.
