I would start with permissions and dependant folders. If there are any file access read/writes involved at the root of the local drive, that is a problem. If there is any involvement of the BDE engine there could be issues there.

Also reproducable within debugging mode: The Delphi appl throws an event, but as soon as the Progress side response via method calls, the Delphi application terminates without any warning, any happening, etc. Even when Delhi debugger is attached the the running process, I could not see any Acess Violations, and I also do not see the 'OnClose' being passed. I searched via google to find any 'known issues' on 'delphi automation Vista', but does not find any information an this topic. Information: Delphi 7, Progress 10b, Any ideas on how to catch this problem would be welcome.
