Attachment: d4001-1a-test.diff
Attaching a patch - d4001-1a-test.diff - which adds a (not yet enabled) test
for this bug. I've written test cases for all the problematic cases I've found.
All the queries (except two which I included for completeness and marked with a
comment) fail without the fixes suggested in an earlier comment. The test
checks both that the correct rows are returned and whether or not the queries
are flattened.
