I have exact the same problem with Rhythmbox 0.11.2, and I must put the Rhythmbox volumen to 50%.

It sounds like the speakers were damage, buy with Exaile, Mplayer, Banshee and Rhythmbox there is no problem. It comes when the song has very high tones. So I think thats Rhythmbox problem, not from the audiocard.
