I got this issue with Alacarte 0.11.3-0ubuntu2. 1) I installed 'lyx' from local DVD (repositories) using synaptic.
2) It didn't show up in the Office menu entry (Applications-> Office). Not there. 3) Yet it was there when you "Edit menus" (alacarte). 4) To make it show up I had to untick it from the "Main Menu" editor app (alacarte), then "Close", then edit it again and tick it.
