Today I saw this on unmaximized windows for the first time, using the Mist theme. Pictures are attached. (Look at the terminal windows.)
I use the traditional UNIX style of focus switching (i.e., whichever window the mouse is over has focus), and I've found that switching focus by just moving back and forth between windows can repro this. Interestingly, it seems that mousing over the title bar's buttons does not.
