I'm still experiencing this problem on an up to date hardy. It doesn't seem to occur when running with the --debug flag so I can't provide useful logs. It does however display [0x173e710] [volume_changed_cb] rb-player-gst-xfade.c:916: unexpectedly got a volume change for stream file:///path/filename 1.000000 (not fading) on every transition.
What I have noticed however is that when it happens the time indicator of the main rhythmbox window indicates for example 5:10 of 5:10 whereas it shows 2:34 of 5:10 in the notifications area when hovering over the icon. I can't provide a way to reproduce it. It occurs from time to time without any obvious cause. Sometimes it plays days on end without occurring and when it does occur and replay the song it always goes to the next one when it finishes. So far it has only occurred without the debug flag in shuffle mode with crossfading enabled.
