http://www.cs.brown.edu/~sk/Publications/Books/ProgLangs/PDF/plai-2006-01-15.pdf Evince opens the PDF fine, and the contents side bar shows the contents tree fine, as well. However, none of the elements in the sidebar have page numbers, and clicking on any of them jumps to the first page in the PDF.
