If the drive is already mounted, then is there any clue in the mount
table configuration? Note that HAL is what is responsible for mounting
and unmounting the CD drive. Normally it launches gnome-mount and this
will autolaunch programs as per specified in the GConf keys defined in
desktop/gnome/volume-manager. You can use gconf-editor to see these
settings, and verify that they are reasonable.

If the problem is with HAL, running lshal will show you what devices
are currently recognized by HAL. If it seems that it is incorrectly
showing the drive as being mounted when its not, then I'd file a bug
against HAL and attach the lshal output.

Another test you can run is to see if it works any better when you log
in via the root user. There have been issues in the past caused by
the fact that non-root users do not have permissions to do certain
things needed by removable media.
