GNOME's menu specification is defined on freedesktop.org :

Desktop entry spec :
    http://www.freedesktop.org/wiki/Specifications/desktop-entry-spec

Menu spec :
    http://www.freedesktop.org/wiki/Specifications/menu-spec

Mime type spec
    http://www.freedesktop.org/wiki/Specifications/shared-mime-info-spec


Adding a menu item without using the alacarte menu editor can be done by simply
adding a .desktop file to /usr/share/applications (for all users) or for a 
specific user to ~/.local/share/applications.
