It appears to be random but I believe it may be related to using Alt-Tab to switch between apps on the client machine. This means the only way to disconnect is to right-click the Unity panel icon and select Exit. Minimise is impossible until reconnected. Sometimes the bar appears when using other apps too for some reason, which may or may not be related.
