It can happen from the network applet so long as the network actually 
exists and enterprise menu comes up to configure the network.

The bug is present with both Unity and GNOME-Shell.

  If you connect to a WPA2-Enterprise network and do not choose a CA
  cert, you will be presented with a modal dialog in which neither
  option can be selected with the mouse or keyboard. The mouse focus is
  still on the window underneath.
