There are some other programs that cause the same problem in the UI, then it is 
NOT (probably) caused by LibreOffice, although occur more frequently with it. 
I've had the same problem using PGAdmin.

Bug description:
  After opening 3 or 4 LibreOffice files by double clicking filename in
  Nautilus, toolbar (and sometime menus) is buggy.

  Steps to reproduce the problem:
  1. Install Ubuntu 12.10
  2. Open Nautilus, double click a ODT/ODS/ODP/DOC/DOCX/etc file to open it in 
LibreOffice
  3. Repeat the operation 3 or 4 times
  4. The problem occurs.
