This problem is mostly happening for me when trying to access an application on a different workspace by clicking the application's icon in the unity launcher.
It happens very often with skype, but also terminal is now affected. If I try to access terminal by clicking on the according icon in the launcher, the window is lost. If an application like GIMP or NETBEANS opens a dialog, it might also be lost if you switch to a different workspace before the dialog opens.
