I can't remember if this happened only with Ubuntu 12.10, or if I saw it
on 12.04. I removed 12.10 and reinstalled 12.04 some time on 2012, but
I'm not sure if that would have been before or after I reported this
bug.

I don't really know what this questions means. The bug describes what I
did. ie. I did not press anything that is not mentioned. That is, I did
not press alt (or anything else) before it appeared.

No. The only thing that I pressed after it appeared was Esc in order to
make it go away.

  command field appears when playing a movie

Status in “totem” package in Ubuntu:
  Incomplete

Bug description:
  When playing a movie, a command field (that looks like the Alt-F2
  command field) appears after about 10 seconds.

  It is possible to hit Esc once to make it go away, but it comes back
  after about another 10 seconds.
