This is happening to me running 12.10, kernel 3.5.0-26. I was using the Nvidia driver and thought switching to Nouveau might help but it was the same. So this does not seem to be an Nvidia related issue. At least in my case I am thinking this could be a software reaction to faulty hardware: I am running on an XPS m1530 and for a while even the BIOS POST was not visible. The screen was lit, but remained black. It seems to be starting again, but now I get fatal IO errors showing up in /var/log/syslog and get booted out to GDM. Just before this happens the screen either freezes, goes all blocky or gets crazy rainbow colored banding.
Another datapoint for consideration. -- You received this bug notification because you are a member of Desktop Packages, which is subscribed to nvidia-graphics-drivers in Ubuntu. https://bugs.launchpad.net/bugs/999191 Title: X crashes with gnome-session: Fatal IO error 11 Status in X.Org X server: New Status in “nvidia-graphics-drivers” package in Ubuntu: Incomplete Bug description: It started happening after upgrading to ubuntu 12.04. Several times a day, the gnome session crashes and drops me to GDM. This happens with Unity and gnome shell. It doesn't happen with unity-2d.
