There have been many changes in Ubuntu since that time you reported the bug and your problem may have been fixed with some of the updates. It would help us a lot if you could test it on a currently supported Ubuntu version. (Only the Cancel and Done buttons appear, no textboxes) This did work on Quantal. After debugging, the line it seems to get blocked on is "if (priv->ready)" in method empathy_account_settings_check_readyness from empathy-account-settings.c (the debug log before is printed, but the log after - neither the one in the if nor the one after the closing bracket - is not). Let me know if I can help you with any further details. As a note: the symptoms are similar to the ones of bug #1147639, only that one is for the IRC provider.
