Could you try this with NetworkManager running in debug mode? It would
be useful to have all the logs from NetworkManager.

To do this, you can edit /etc/init/network-manager.conf; to add --log-
level=debug to the Exec line.

The logs will be in /var/log/syslog.

  Does not refresh the list of contexts from ofono when a new context is
  added

Status in “network-manager” package in Ubuntu:
  Incomplete

Bug description:
  Add a context via the command line scripts or via the QML api.
  A context added signal is issued by ofono (per dbus-monitor --system 
"interface=org.ofono.ConnectionManager" and QML signal)
  The list of connections in network manager does not change (per nmcli con)
