Title:
  Errant volume control

Status in pulseaudio package in Ubuntu:
  Incomplete

Bug description:
  I have a problem I am having is that I have the volume set to mute and
  when I turn on Firefox suddenly i am getting BLASTED out of my seat
  when there suddenly is max volume for say COMMERCIALS which I despise.
  This is all the way across the board with anything I do on line with
  the volume muted. Some applications also override the volume control.
  Likewise I have not been able to get LMMS to work at all. it is
  properly installed but I cannot get any configuration to work allowing
  me to get sound from this one app. I think the problem is related.
  Some how some apps like Firefox can override the volume control. This
  also is quite annoying when I start the computer ands the volume has
  been muted yet it will still make teh bongo start sound and others
  even with the volume turned to mute.
