Had the same problem on a Dell PowerEdge R730 with Ubuntu 16.04LTS.
Installing module mgag200 as suggested by Ivo Straka's (approach n. 1) worked 
for me.

But as Giorgio Signorini comment #61, after installed mgag200 module and set 
xorg driver to mgag200 and restart X.
In /var/log/xorg.0.log, there will have somes errors like "Could not load 
driver maga200", and finally fall back to xserver-xorg-video-mga if installed.

Actually, in xf86-video-mga
1.6.5(https://cgit.freedesktop.org/xorg/driver/xf86-video-mga/) has
added support for g200e, while in ubuntu 16.04 only with 1.6.4
version(https://launchpad.net/ubuntu/+source/xserver-xorg-video-mga),
and 1.6.5 only for Zesty(https://launchpad.net/ubuntu/+source/xserver-
xorg-video-mga/+publishinghistory).

This is because of dependency problem, or not? Since I'm new to linux,
maybe who could build xf86-video-mga 1.6.5 in ubuntu 16.04 or give some
ideas about how to build?
