Bug description:
  this occurs on a system that has DisplayPort 1.2 daisy-chained
  monitors, that are connected via a DisplayPort 1.2 compliant KVM
  switch. Crashes usually occur at the moment the switch disconnects the
  monitor chain, or restores the monitor chain, at which time the KVM
  may be suddenly reporting unusual resolutions for the DP monitor(s).
