Randomly when using a USB audio device my headphones will start
screeching at a deafeningly loud volume.

This seems to be a documented bug in pipewire with a fix committed last week.

I'd say this is pretty major. The volume is not controllable and using
headphones it was painfully loud causing my ears to ring for several
minutes.
