It also doesn't appear in the Unity panel while open. This does not happen every time the program is launched. It seems that this occurs when directly opening a file (e.g. double clicking a file in Nautilus or downloading a .ppt or .doc in Firefox and clicking "Open" instead of "Save") Unmaximising shows the window title and buttons, but the icon still doesn't appear in the Unity panel.
