I just did a new installation of Xubuntu 11.10, and then upgraded all the packages. The resulting Firefox version is 11.0.
I changed the contents of my /etc/defaults/locale file to LANG="en_GB.UTF-8" LC_TIME="en_DK.UTF-8" then logged out and back in again. Now, when I print a page from Firefox, A4 is the paper size used, which is correct, but when I do a print-to-file, the default paper size is still letter. So this bug has not yet been completely fixed. By the way, it's coming up on the bug's tenth birthday.
