Well FWIW I am fairly interested in accessibility issues and find this screen 
reader pretty difficult to use.

Is there documentation somewhere? I tried searching the web and the code and 
can't really find my way around to know how to use it.

So far I can't do a couple of basic things:
- can't get the quick setting bar (what's the proper name I wonder?) 
- can't swipe to other home screens to get to settings

That helps a bit... I can unlock the screen now and navigate around... though 
tab/shift tab are swipe right and left respectively, not swipe up/down it seems.

I can't get off the "I'm thinking of..." screen though. Any clues on how to get 
to the screen where I can change settings? Also can't pull down from top to get 
the "quick settings" where I could get to settings as well.

I'll see if I can read the screen reader source and figure it out...

Now, some answers to your questions:
* your profile is in /data/b2g/mozilla/*.default/
** in this directory, you have prefs.js which holds your user
preferences. You need to edit this with b2g stopped (after running "stop
b2g")
* indexedDB files are in /data/local/storage/persistent for current
versions (maybe /data/local/indexedDB for older versions, I don't
remember exactly)
* the settings are stored in an indexedDB db, there is no rea easy way
to edit it
