Unable to create document in DMS

Using rev. 13323.
Steps taken:
1) right click on a folder, select "New Document"
2) Select file to use and enter Subject
3) Click on 'Save'
Instead of saving a javascript error is thrown "txt has no properties" at line
3311. Looks like it is unable to find the 'commentPopUpTextArea'. Happens in
FF 2, IE 6, 7.
