Even though server issues Cache-Control: no-cache FireFox ignores this header and does cache the content.
In order to replicate: You need a web server that issues Cache-Control: no-cache with all successfull responses. Have a long file there and page with link to download this file. Open the page in the browser and click on download link SaveAs dialog will appear, wait for some seconds and close it with Cancel. Click on the link again.
