Convert Hosts table to use server-side paging.
The API already supports paging (page_size, from, etc).
Note that sorting and filtering does not work yet (there's a patch on filtering
but it is not integrated to the build yet), but don't worry about that for now
(it's ok for filtering/sorting to break for the time being).
