Not sure if these are the cause but worth checking:

1. Check that the configuration of all the cluster nodes is consistent and
correct (sometimes if 'salt' or other similar tools
    are themselves mis-configured or malfunctioning, some of the cluster
nodes may end up incorrectly configured).
2. Check that HDFS is working properly and not corrupted.
