Actually, let's get this PR merged.
I'll PR my stuff separately, which will duplicate a little bit of yours.
Suggest you review mine, and once that is merged then add cross-references from
yours to mine.
The division in my mind is:
* users deploying stuff will want to look at the ops guide - they shouldn't
have to look inside the entity implementation; any suggestion that they should
will be scary for them.
* entity developers will care about how to test + fix their *own* entity
implementation. This guide is excellent for that.
