You can see example files in ctakes dictionary-lookup-res.  Start with the 
simplest 2:

*note: there is a missed directory in the examples paths in the example xml 
files.  To use them directly you'll need to add "bsv/" to the .bsv paths in the 
xml files.  I thought that I had fixed that ...
