Rebased onto the latest master.
The profile UI in master had changed since this PR was first posted.
Reworked the query overview information to use the much better style added
recently in master.
* The "Queued" field for duration appears only if queues are enabled.
* The overview line includes query cost and queue if queuing is enabled.---
