Add a "Was generative AI tooling used to co-author this PR?" section to
.github/PULL_REQUEST_TEMPLATE.md, mirroring the one in apache/flink.
Contributors disclose AI tooling by checking the box and filling in a
Generated-by: <Tool Name and Version> line, and remain responsible for the
correctness of every change regardless of tooling used.
