I've applied the patch as it is an improvement over previous behaviour. 
However, we should really extract the title of the document from the file and 
use that (or at least an encoded version). 

I've reduced the priority of this issue as the current behaviour is an 
improvement on the original behaviour.

(NOTE: please also update any documentation that needs to change as a result of 
these changes, I did so when commiting this patch)
