Summary: Refactor KeyValueStore hierarchy so that
MeteredKeyValueStore is the outermost store

MeteredKeyValueStore is currently not the outermost store. Further it needs to
have the inner store as {{<Bytes, bytes[]>}} to allow easy plugability of
custom storage engines.
